Maternity Grant Program

The Maternity Grant Program is designed to help offset the costs of the prenatal journey and delivery of your newborn by providing you with extra HSA funding to cover maternity/delivery expenses.

How Does It Work?

Step 1: Let us know you are pregnant 
Submit your request to Employee Hub ([email protected]). In the subject line of your email, add: CONFIDENTIAL: Maternity Grant Request and attach the completed Maternity Grant Request Form. Please do not include other personal information.

Step 2: We add extra funds to your HSA
The amount of the grant award is $2,000 if all qualifications are met. If eligible and approved, the grant will be deposited directly into your HSA.

Who is eligible?
  • Be enrolled in a medical plan through the company and enrolled in Optum HSA
  • If your spouse, domestic partner or dependent child is pregnant and you meet the criteria above, they must be enrolled in one of our HDHPs to be eligible for the grant.
  • You must be actively employed at the time of the pregnancy to be eligible. You must apply +/- 9 months of the birth of child.
Grant Guidelines
  • To be considered, ask your medical provider to complete the Maternity Grant Request Form and submit it to HR Shared Services
  • This is a grant; it does not need to be repaid
Grant Award

The amount of the grant award is $2,000 if all qualifications are met. Your annual seed money, grant award, and personal HSA contributions cannot exceed the annual IRS limit. If eligible and approved, the grant will be deposited directly into your HSA

Who is eligible?

Any benefits-eligible employee (who is covered by an employer-offered health plan [ours or spouse’s or domestic partner’s]) may receive expert assistance and services from Carrot. Covered dependents are not eligible for family-focused care and travel expense reimbursement.
