Medical Leave

A medical leave of absence is taken for your own serious illness or injury (non-work related). A leave of absence request is required if you will be away from work for four or more consecutive business days.

Required Documents (Forms)

There are specific forms and/or documentation required by the Company or Federal law to be approved for a leave of absence. Depending on your leave type, your doctor may be required to provide information.

Begin this process as soon as possible to avoid delays in payment and/or approval for leave.

Submit your request for leave in Workday.
Login Go to the Quick Action buttons at the top of your Workday homepage and
click the Request Absence buttonSubmit request and attach required forms (if available at this time)

Employee Hub will guide you through your leave of absence, handled with confidentiality, from beginning through your return to work. Once your leave is approved, your manager and HR Team will be notified that you’ve been approved for a leave of absence and the timeframe of the approved leave.
